We are currently hiring for:

• PRN MSWs to manage a small caseload and support overflow visits
  
  
  

Position Summary

As a Hospice Social Worker, you’ll provide in-home psychosocial assessments, counseling, resource coordination, and emotional support to patients and their families. You’ll work as part of an interdisciplinary team, ensuring compassionate, comprehensive, and coordinated end-of-life care.

Qualifications

• Education: Master’s or Doctoral Degree in Social Work from an accredited institution
• Licensure: Active LMSW or LCSW license in the state of Texas
• Experience: Minimum of 1 year in hospice or a related healthcare setting
• Skills: Strong interpersonal communication, counseling, and knowledge of community resources
• Certifications: Current CPR certification required
• Transportation: Reliable vehicle, valid driver’s license, and current auto liability insurance
