Position Summary:
The Intake Coordinator is responsible for managing the intake process for new clients seeking home care services. This includes coordinating with referral sources, gathering necessary documentation, verifying insurance and eligibility (e.g., Medicaid, MLTC), and ensuring a smooth transition from referral to service delivery. The Intake Coordinator plays a key role in maintaining positive relationships with families, clients, hospitals, and other referral sources.
Key Responsibilities:
- Receive and review referrals from hospitals, insurance plans, MLTCs, social workers, and other sources.
- Contact patients and families to explain services and confirm care needs.
- Collect and verify required documentation including Medicaid numbers, physician orders, UAS assessments, and other eligibility information.
- Coordinate with internal departments (e.g., Nursing, HR, Scheduling) to ensure timely onboarding of new cases.
- Input all client information accurately into the agency’s system and maintain compliance with DOH and agency standards.
- Follow up with referral sources on missing documentation or clarifications.
- Communicate clearly with families and MLTCs regarding start of care, aide assignment, and service coverage.
- Ensure cases are assigned and opened within required timeframes.
- Track pending cases and report regularly to management on intake status.
Requirements:
- High School Diploma or GED required; Associate’s or Bachelor’s degree preferred.
- , or case management (NY home care experience strongly preferred).
- Knowledge of Medicaid, MLTC plans, UAS assessments, and home care eligibility criteria in New York.
- Strong organizational and communication skills.
- Proficiency in Microsoft Office and systems (e.g., HHAeXchange, ASANA).
- Ability to manage multiple referrals and prioritize workload in a fast-paced environment.
- Bilingual (Spanish, Russian, or Creole) a plus.
