Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

Grounded in the Jewish values of dignity, compassion, and community, the San Francisco Campus for Jewish Living, fondly referred to in the past as "the Jewish Home," was founded in 1871. Today, 150 years later, it continues to build upon its legacy of enriching the lives of older adults.

Job Summary:

Director of Quality of Life/LCSW is responsible for the oversight and coordination of social services within SFCJL. This role ensures that all social work practices align with the organization’s mission to deliver compassionate, patient-centered care while complying with the regulatory standards from the CMS, CDPH, and Title 22. Areas of responsibility include discharge planning, supervision of social services staff, behavioral management programs, program development, professional outreach, translation services and public benefits. In addition to ensuring the best quality of life for all patients/residents, the director ensures timely and accurate psychosocial assessments, care coordination, discharge planning, and follow-up care.

Key Responsibilities:

  • Manage social services support for the team in post-acute, long-term, and geriatric psychiatric settings, ensuring regulatory compliance and high-quality standards of care.
  • Oversee discharge planning and care coordination.
  • Supervise psychosocial assessments.
  • Manage the provision of counseling and support to patients and families.
  • Partner with other disciplines to promote clinical excellence and ensure effective patient advocacy.
  • Ensure compliance with CMS Conditions of Participation, CDPH standards, and Title 22 regulations related to patient rights, discharge, and psychosocial documentation.
  • Coordinate with families, legal representatives, and community-based programs to facilitate aftercare planning, housing, and access to behavioral health services.
  • Develop, review, and update policies and procedures for social services operations and integrate quality improvement initiatives.
  • Participate in survey readiness, data monitoring, and hospital-wide performance improvement projects.
  • Participate in unit-based projects aimed at enhancing patient experience.
  • Collaborate with the healthcare team to identify areas for improvement and implement solutions.
  • Perform other duties as assigned.

QUALIFICATIONS/JOB REQUIREMENTS:

EDUCATION:

  • A Master’s in Social Work is preferred. LCSW license is required.

EXPERIENCE:

  • Must have 10+ years of experience in social services and discharge planning
  • Excellent interpersonal and communication skills.
  • Strong understanding of CMS Conditions of Participation, CDPH standards, and Title 22 requirements for social services and discharge planning
  • Knowledge of MS Office, specifically Word and Outlook.
  • Demonstrated leadership, organizational, and communication skills.
  • Knowledge of community mental health systems, legal requirements under the Lanterman-Petris-Short (LPS) Act, and discharge planning for high-risk psychiatric patients.

CERTIFICATIONS/LICENSURE

  • CPI certification or other Assaultive Behavior Management program certification within 3 months of hire.
