We’re seeking an empathetic, detail oriented, and organized Intake Coordinator to join our growing team and serve as the first point of contact for patients, families, and referral sources.

Position Overview:
The Intake Coordinator plays a critical role in the admission process, coordinating referrals, gathering essential documentation, and ensuring a smooth and compassionate experience for patients and families during a sensitive time. You will collaborate closely with our nursing, admissions, and administrative teams to ensure timely, compliant, and efficient intakes.

Key Responsibilities:

  • Receive and process referrals from hospitals, physicians, SNFs, and families.
  • Obtain and verify insurance information, demographics, and clinical documentation.
  • Coordinate timely nurse assessments and start of care visits.
  • Communicate effectively with referral partners, patients, families, and internal departments.
  • Ensure all required intake documentation is completed accurately and timely.
  • Track and follow up on pending referrals, authorizations, and orders.
  • Maintain accurate logs and status reports on all referrals and admissions.
  • Participate in staff meetings and contribute to improving intake workflow.

Qualifications:

  • Prior hospice or healthcare intake/admissions experience.
  • Familiarity with hospice eligibility criteria and Medicare guidelines.
  • Excellent communication and customer service skills.
  • Strong attention to detail and ability to multitask in a fast-paced environment.
  • Knowledge of EMR systems and HIPAA compliance.
  • Bilingual (English/Spanish) a plus, but not required.
