Join us as a Patient Care Coordinator and play a critical role in ensuring that patients receive timely, accurate, and compassionate support during their care journey.

What You’ll Do:

  • Manage and track key patient documents, including face-to-face forms for physician signatures
  • Gather hospital records and key paperwork such as H&P, physician orders, and ROC notes
  • Make pre-admission, follow-up, and discharge calls to ensure accuracy and continuity
  • Monitor hospitalized clients and notify liaisons of discharges for timely care coordination
  • Provide administrative support that helps our nurses, therapists, and care teams do their best work

Qualifications:

  • High school diploma required; college degree preferred
  • At least one year of experience in a healthcare or fast-paced office setting
  • Strong verbal and written communication skills, with a phone presence that builds trust
  • Typing speed of 50+ WPM and comfort with navigating electronic medical records
  • Familiarity with medical terminology and home care processes is a plus

This role is a great fit if you’ve worked as: Health Unit Coordinator, Insurance Verification Specialist, Medical Records Clerk, Intake Coordinator, or Patient Services Representative—especially in long-term care or home health settings.
