Pilsen Wellness Center, Inc. is a community-based not-for-profit agency that provides culturally sensitive services to a multicultural population with limited financial resources. The organizations mission has remained providing quality human services to economically disadvantaged children and families. The agency is comprised of five divisions - Mental Health, Substance Abuse, Youth Services, Early Intervention and Education which are currently supported through 17 different locations.


POSITION SUMMARY

We are seeking a Mental Health Professional (MHP) to join our PATH (Projects for Assistance in Transition from Homelessness) team, funded by SAMHSA. PATH is a federally funded initiative, designed to connect individuals with SMI who are experiencing homelessness to mental health, substance use, housing, and support services. Services may include outreach, screening, treatment referrals, and housing assistance. The ideal candidate will work with individuals experiencing homelessness and living with Serious Mental Illness (SMI)—including schizophrenia, bipolar disorder, and major depression.

WORK SCHEDULE

The work schedule available is: Monday - Friday 9:00am-1:00pm

DUTIES AND RESPONSIBLITIES

This individual participates in the development and implementation of individual service plans in order to increase a participant’s independence skills in daily living, economic self-sufficiency, and community integration which may include an array of services and interventions designed to provide treatment, habilitation, training and other community integration services.

EDUCATION and EXPERIENCE
  • Experience with field-based outreach and engagement
  • Skills in crisis de-escalation, harm reduction, and Housing First principles
  • Cultural humility and the ability to build trust with diverse populations
  • Knowledge of mental health systems, housing resources, and public benefits
  • Flexibility to work in shelters, encampments, and street settings
  • Strong documentation and collaboration skills

This individual must have a Bachelor Degree in Psychology or a related field OR 5 years’ experience in the Mental Health Field. Familiarity with Medicaid Rule 132 desired. Valid Driver's License required. Bilingual (Spanish) required.

COMPENSATION:

$22.00 USD per hour
