H.E.R.S. (Helping Empower Reentry Services) is dedicated to supporting individuals and families in Monterey County who have Medi-Cal. Our mission is to provide comprehensive services that empower our clients to rebuild their lives and achieve lasting success.

Position: Enhanced Care Advocate

Location: Hybrid in Monterey County

Compensation: DOE

Enhanced Case Management - Role and Responsibilities:

Personalized Case Management: Our dedicated Case Managers work one-on-one with clients to create individualized care plans that address their specific needs.
Mental Health & Substance Use Support: We connect members with mental health services, substance use counseling, and support groups to help them on their journey to recovery.
Healthcare Navigation: Assistance in accessing medical, dental, and behavioral health services, ensuring members receive the care they need.
Advocacy & Legal Support: We advocate for our clients' rights and help them navigate the legal system, including parole and probation requirements.
Employment & Education Assistance: Guidance in job search, resume building, and connecting with vocational training and educational opportunities.
Resource Coordination: Connecting clients with local resources, including food pantries, clothing, transportation, and more.
