One part-time (25 hours per week) position with an office in Council Bluffs. This position will support Pottawattamie, Mills, Cass, Harrison, and Shelby counties southwest Iowa and,
One part-time (15 hours per week) position working from a home-based office in the Sioux City area. This position will support Woodbury, Ida, Monona, and Harrison counties.

Rate of Pay: $21.88 - $24.47 per hour

Contract work available in lieu of regular employment

About the Role

Firefly is seeking a dedicated Maternal Health Social Worker to provide psychosocial support to clients enrolled in our Title V Maternal Health Program. You’ll work directly with families through home and office visits, ensuring they have the care, resources, and support needed for healthier pregnancies and brighter beginnings.

This is a community-focused position requiring local travel, strong interpersonal skills, and a passion for maternal and infant health.

What You'll Do

Provide psychosocial services, listening visits, and non-medical care coordination
Interpret depression screenings and assist with Medicaid presumptive eligibility applications
Develop and manage individualized care plans for clients
Facilitate referrals to Firefly programs and external community partners
Maintain accurate electronic health records and assist with compliance reviews
Communicate professionally with clients, providers, and partner agencies
Maintain confidentiality and promote culturally responsive care

Qualifications

Required:

Bachelor’s degree in Social Work (BSW) - Degree in Social Work required by state contract
Valid driver’s license, personal vehicle, and current auto insurance
Reliable transportation and willingness to travel throughout western Iowa
Ability to build trusting relationships and work independently
Strong communication and documentation skills
Understanding of maternal/infant mental health and development
Must pass background check, and state/federal requirements

Preferred:

LISW (Licensed Independent Social Worker)
Experience in maternal health, public health, or community outreach
Bilingual abilities a plus

Working Environment

Home visits and travel required (in all weather conditions)
Flexible schedule; some evening and weekend hours may be necessary
Exposure to various home environments and health-related conditions
Frequent sitting/standing; occasional bending, reaching, and stair-climbing

Physical requirements

While performing the duties of this job, the contractor will frequently sit and stand. The contractor is occasionally required to stoop, kneel, climb steps, get down on the floor, and reach forward and above the head. The contractor regularly interacts with staff and the public and meets multiple demands and deadlines from several people. The contractor must move from place to place, building to building, and travel to attend meetings. Requires corrected vision and hearing within normal ranges.
