Position Overview
The Licensed Clinical Social Worker at Harvest Family Health Center is part of our progressive movement towards holistic healthcare. The Licensed Clinical Social Worker would be a part of a team providing integrated behavioral health consultation to children, adolescents, adults, and families, in order to improve psychosocial functioning.
This position may include addressing mental health and substance abuse conditions, health behaviors (including their contribution to chronic medical illnesses), life stressors and crises, stress-related physical symptoms and ineffective patterns of health care utilization. The Licensed Clinical Social Worker would also assist the team of primary care physicians in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education.

Essential Tasks
- Assess the clinical status of patients referred by primary care providers and outside referral sources to develop a patient focused plan of care for substance use disorder and/or mental health treatment.
- Incorporate chronic disease management into overall treatment goals.
- Use evidence-based assessment screening tools with patients to assist with identifying concerns and outcome measurements.
- Consult with medical providers as necessary, and refer cases to specialty, case management, and other behavioral health/substance use providers as appropriate.
- Work with primary care team to treat and manage patients with chronic mental health problems and/or substance use disorders efficiently and effectively to improve clinical outcomes.
- Educate staff via in-services on related behavioral health topics when needed.
- Teach treatment enhancement techniques using non stigmatizing language and empathy to patients, families, and staff.
- Accurately record patients past medical history, social and medication history, records therapy provided, and referrals in medical record, according to established format. Documentation will be completed in a timely manner according to protocol.
Experience and Education
- Master's degree in social work, psychology, or related Behavioral Health field (from regionally accredited and approved institutions).
- Demonstrated working knowledge of brief, evidence-based treatment modalities.
- Ability to manage the pace of primary care and work with an interdisciplinary team.
- Minimum one-year field experience in primary care setting preferred, but not required.
- Must have the following skills, intermediate level computer skills, strong interpersonal skills including the ability to demonstrate empathy, effective communication skills, and analytical skills.
- BLS Certification.
Schedule
Monday, Tuesday, Wednesday, Friday - 8 am – 5 pm
8-hour shift, day shift
Thursday, 11 am - 8 pm, 8-hour shift, night shift
Physical Requirements
- Individuals may need to sit or stand for long periods of time, occasional bending, stooping, lifting including of patients, reaching forwards and overhead.
- Possibly requires walking primarily on a level service for periods throughout the day.
- Both proper lifting techniques and frequent computer work are required.
- Must be able to travel freely in the local area; between clinics, hospitals, health departments, etc.

License/Certification
- Fully licensed LCSW, licensed to practice in the state of North Carolina, is preferred.
- Associate Licensed LCSW will be considered.
- Recent graduate will be considered.
- Valid North Carolina Driver's License.
- BLS Certification.
Base Pay Overview
The starting pay for this position is $65,520.00. Please keep in mind that actual salary will vary based on various factors such as qualifications, skills, competencies, and proficiency for the roles.
