Social Worker- PART Time- Temp

Job Description:

Provides support to the residents and families in coordinating residents needs to ensure residents needs and expectations are met during their stay at Encore at Boca Raton and their expectation in the future. Work closely with the interdisciplinary team in coordinating the care plan and discharge plan.

Responsibilities Include:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Qualifications/Requirements:

• BSW or MSW preferred, or Bachelors degree in related field
• At least one year experience in Social Work field required
• Long term care and/or sub-acute setting preferred
• Ability to work flexible schedule
• PCC experience preferred
