About the Role
We’re seeking a Qualified Mental Health Professional – Community Services (QMHP-CS) to join our growing team. In this role, you will provide community-based mental health rehabilitation and targeted case management services to individuals aged 6 and older. Services are delivered primarily in-person (75%) across home, school, and community settings, with the flexibility to conduct 25% of sessions virtually.
This position offers ongoing clinical supervision, opportunities for community outreach, and a chance to truly make a difference in the lives of those we serve.
What You’ll Do
- Provide skills training, psychosocial rehabilitation, and targeted case management in alignment with the Texas Resilience and Recovery (TRR) model
- Meet clients where they are – in their homes, schools, and community locations
- Accurately document all services using an Electronic Health Record (EHR) system in compliance with Medicaid standards
- Collaborate with clients, families, caregivers, and community partners to support personalized recovery goals
- Participate in weekly clinical supervision, staff meetings, and ongoing professional development
- Assist with community outreach and build relationships to support client referrals
- Contribute to treatment planning and coordinate care with licensed providers
- Complete necessary assessments including CANS (Child and Adolescent Needs and Strengths) and ANSA (Adult Needs and Strengths Assessment)
What We’re Looking For
- Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, or a related human services field
- Must meet Texas HHSC qualifications to serve as a QMHP-CS
- 6+ months of experience in mental health rehabilitation or targeted case management (preferred)
- Comfortable working in community-based environments and with diverse populations
- Familiarity with EHR systems and Medicaid-compliant documentation practices
- Excellent time management, communication, and organizational skills
- Independent, dependable, and collaborative with clinical leadership
Preferred Qualifications (Not Required)
- CANS and/or ANSA certification
- Training in Nurturing Parenting Program or Texas Wraparound
- Bilingual (Spanish/English) is a plus
- Knowledge of TRR guidelines, Medicaid documentation, and recovery-oriented care models
