Elmwood Healthcare's Home-Health Division, Elmwood Home Care, is a Medicare certified, Joint Commission accredited organization providing skilled and non-skilled home-health care to Rhode Island and Massachusetts communities, by way of Agency branch locations in Providence, RI, South County, RI and Taunton, MA.

CARE COORDINATION MANAGER (CCM)
Responsibilities:
- Understands and adheres to the established policies, procedures and operational practices of Elmwood Home Care.
- Provides direct supervision, direction and support to Care Coordination personnel;
- Performs performance appraisal and review of department personnel;
- Provides counseling and/or disciplinary action to department personnel when indicated;
- Provides initial and ongoing education to all department personnel;
- Works collaboratively with the Leadership team and Governing Body, acting to bridge operational viability and coordination of patient care to maintain integrity and safeguard patients and staff;
- Creates, oversees and maintains the schedules of designated clinical staff in correlation to patient needs and staffing variances;
- Enters scheduling data to create and maintain accurate orders in accordance with care plans;
- Works interdisciplinarily to create a consistent flow of high-quality patient care;
- Contacts care-providers, clinicians and patients with updates regarding day-to-day scheduling matters;
- Receives, processes and facilitates referrals in accordance with the systematic guidelines set forth by Elmwood Home Care;
- Ensures the Agency only accepts those patients whose needs can be met by the services it provides;
- Monitors and responds to electronic referrals via electronic referral systems;
- Verifies and coordinates insurance benefits to safeguard reimbursement of services provided;
- Counsels the patient/representative and family/caregiver meeting communication and coordination of care needs.
- Works closely with Sales Teams to satisfy mutual goals and satisfy the needs of the Agency;
- Generates reports and data regarding scheduling, care coordination and staffing as designated;
- Monitors, modifies, develops and implements care coordination systems and processes based on personnel and leadership feedback;
- Participates in utilization review of medical records as assigned;
- Acts in accordance with the Elmwood Home Care Employee Code of Conduct;
- Ensures the safety, security, integrity and confidentiality of patient and staff information in accordance with HIPAA and agency standard regulations;
- Upholds the standards of patient satisfaction, customer service and quality of care set forth by the Agency;
- Demonstrates an understanding of the operational flow of a Medicare-certified home-health agency;
- Demonstrates an understanding of the post-acute and transitional-care processes;
- Understands and adheres to policies, procedures and regulations set forth by the agency;
- Understands and adheres to policies, procedures and regulations set forth by local, state and federal governments.
Requirements:
- High School Diploma or GED Equivalent
- Associate’s or Bachelor’s Degree or Experience-Based Equivalent Level of Higher Education
- 1-2 Years of Prior Industry Experience in Healthcare Administration or Operations
- 1-2 Years of Prior Healthcare Experience
- Proficiency in the Application of Moderate-Level Computer and Technology Skills (MS Excel, Cloud-Based Softwares)
- Proficiency in the Understanding and Comprehension of Applicable Local, State and Federal Regulatory Standards
