About the Pope Francis Center: Pope Francis Center exists to provide meals, supportive services, and housing assistance to Detroit community members in need, especially those experiencing homelessness. On an average day, we welcome nearly 200 guests and provide them with nutritious meals, showers, laundry, and access to doctors, dentists, lawyers, and housing providers through our free clinics. We have been serving Detroit’s most vulnerable citizens for over 33 years, and we envision a Detroit without homelessness, where every person has the resources for sustained stability.

Position Overview: The Care Plan Coordinator manages all aspects of resident care-plan logistics and communication—bridging clients, clinical teams, family, and community providers—so that Caseworkers, Residential Coordinators, and Recovery Specialists can devote their time to direct support.

Key Responsibilities:

Scheduling & Referrals

  • Book and confirm medical, dental, psychiatric, and external referrals.
  • Coordinate transportation logistics for off-site appointments.
  • Educate residents on appointment prep and insurance questions.

Client Communication & Education

  • Hold face-to-face and phone check-ins to update residents on care steps and answer questions.
  • Provide basic health education: self-care, medication adherence, and wellness best practices.

Medication Support

  • Set up weekly 7-day pill organizers; track PRN requests and refill needs.
  • Liaise with CHASS pharmacy, security, and residential staff to ensure safe, compliant medication hand-off.

Data Management & Reporting

  • Perform simultaneous data-entry and charting in Caseworthy (or equivalent): intake, service logs, care-plan milestones.
  • Run regular reports on no-shows, referral completions, and compliance metrics.

Interdisciplinary Coordination

  • Serve as the hub between residents, caseworkers, CHASS clinicians, housing advocates, and family contacts.
  • Draft and distribute daily/weekly team agendas and meeting minutes; escalate care-gaps promptly.

Compliance & Confidentiality

  • Uphold HIPAA and organizational policies in all client interactions and data handling.
  • Ensure services comply with PFC/BHC procedures and CHASS clinical protocols.

Process Improvement & Admin Support

  • Partner with Care Plan Manager to refine SOPs, process flows, and policy docs.
  • Onboard/train any future CPCs and support continuous workflow enhancements.

Qualifications:

  • Bachelor’s in Social Work, Human Services, Public Health, or related field.
  • 1–3 years in care coordination, case management, or medical-admin roles—ideally in supportive housing or behavioral health.
  • Advanced organizational and multi-tasking skills; adept with case-management software and Google/MS suites.
  • Strong communicator, comfortable liaising across clinical, security, and program teams.
  • Demonstrated compassion, professionalism, and strict adherence to confidentiality standards.

Job Type: Full-time

Pay: $23.07 - $25.70 per hour

Expected hours: 80 per week
