Job Description:
The Intake Coordinator maintains an ongoing positive relationship with professional and nonprofessional staff of the agency and the community. The Intake Coordinator supports the coordination of clinical operations of the agency, ensuring quality of care. Specific duties include:
- Schedules, coordinates, and manages services rendered to patients and families by nursing and home health aide staff.
- Coordinates social work and spiritual care with Director of Counseling Services to ensure coordinated delivery of care.
- Serves as first line contact for referral of patient care from physician's offices, case managers, discharge planners, and others:
- takes referrals and evaluates patients and families as candidates for hospice and home care, explaining benefits and services;
- maintains referrals and receives & tracks information regarding changes in client status;
- assigns patients and families with Community Nursing Supervisor to nursing staff based on patient acuity and staff availability;
- makes patient visits as needed for aide orientation and/or patient needs as directed by Community Director of Nursing;
- functions as liaison between Regional Hospice and other agencies to ensure continuity of care from admission to discharge;
- ensures the flow of accurate information between primary nurse, physician, home health aides, therapists, testing facilities, and family
- Supports appropriate utilization of clinical services, ensuring compliance with state and federal requirements by assisting in the maintenance of medical records, ensuring accuracy and compliance with regulations and HIPAA.
- Assures business department verifies insurance coverage and benefits and tracks ongoing updates for continued authorization;
- Provides for communication between RN Case Managers, Social Workers, OT, PT, and Spiritual Care, ensuring coordination of patient services at time of admission;
- Collaborates with Community Director of Nursing and Counseling Services to resolve issues and better coordinate agency patient care services.
Qualifications:
2- 3 years' experience working in a fast-paced medical environment, preferably in admissions or intake. Must have excellent customer service and phone etiquette, as this position is the first point of contact for referral sources and families. Must be able to multitask and be extremely comfortable with high call volume. Clinical experience/background, preferably in hospice/home health (i.e., LPN, CNA, etc) is desired!
