About Job
Our organization is seeking a compassionate and dedicated Social Worker/Counselor to join our team. Our program provides short-term residential support and crisis intervention for individuals and families.
What You'll Do:
As a Social Worker/ Counselor, you will play a crucial role in supporting individuals by:
- Completing comprehensive psychosocial assessments for individuals and families, identifying needs related to mental health, substance abuse (alcohol, tobacco, and other drugs), domestic violence, child maltreatment, and trauma.
- Providing direct clinical support through individual, group, and specialty therapy sessions.
- Managing a dynamic caseload from admission to discharge, ensuring tailored care plans and effective interventions.
- Facilitating appropriate linkages to community-based services and advocating for clients.
- Delivering crisis intervention and case management.
- Collaborating closely with the Program Director and other team members to address program needs, drive strategic goals, and ensure positive client outcomes.
- Contributing to quality improvement initiatives and providing qualitative and quantitative feedback to relevant agencies.
- Maintaining accurate documentation of patient histories, assessments, treatment plans, and outcomes.
- Providing backup coverage as needed, including occasional weekend or evening hours.
- Working from a strength-based, family-focused, and client-centered perspective to identify and fortify individual and family strengths.
Qualifications
- Master's degree in Social Work or related field
- Strong patient care and assessment skills
- Knowledge of HIPAA regulations and medical documentation
- Experience with clinical counseling and care plan development
Job Type: Full-time
Pay: $29.00 - $31.00 per hour
