REQUIREMENTS:
To be considered for this position, the ideal candidate must meet the following criteria:
- Have preferred experience in long-term care, though it's not mandatory

Possess strong written and verbal communication skills
Maintain good physical and mental health
Be capable of performing physical tasks, including lifting weights of up to 40 pounds
Follow both written and oral instructions
Have the training, knowledge, and experience necessary for general supervision of responsibilities
REPORTING:
This position will report directly to the Director of Provider Relations.
JOB DUTIES:
The successful candidate will be responsible for the following tasks:
Engaging in the resident admission process to ensure the plan of care is implemented
Attending interdisciplinary team meetings as assigned
Maintaining ongoing communication with family members as needed
Ensuring that appropriate educational training is provided before discharge
Monitoring medication regimens for proper utilization
Acting as a role model for the interdisciplinary team
Fostering a positive, calm, and effective work environment
Identifying trends in noncompliance and making recommendations
Participating in committees as directed by the Director of Provider Relations
Completing any additional duties assigned by the Director of Provider Relations.
