REQUIREMENTS:

To be considered for this position, the ideal candidate must meet the following criteria:

• Have preferred experience in long-term care, though it's not mandatory

• Possess strong written and verbal communication skills

• Maintain good physical and mental health

• Be capable of performing physical tasks, including lifting weights of up to 40 pounds

• Follow both written and oral instructions

• Have the training, knowledge, and experience necessary for general supervision of responsibilities

REPORTING:

This position will report directly to the Director of Provider Relations.

JOB DUTIES:

The successful candidate will be responsible for the following tasks:

• Engaging in the resident admission process to ensure the plan of care is implemented

• Attending interdisciplinary team meetings as assigned

• Maintaining ongoing communication with family members as needed

• Ensuring that appropriate educational training is provided before discharge

• Monitoring medication regimens for proper utilization

• Acting as a role model for the interdisciplinary team

• Fostering a positive, calm, and effective work environment

• Identifying trends in noncompliance and making recommendations

• Participating in committees as directed by the Director of Provider Relations

• Completing any additional duties assigned by the Director of Provider Relations.
