Housing Case Manager - Lydia Apartments

Pay Range: $47,000 to $50,000 annual salary, based on qualifications

Benefits: Full time, benefit eligible

Schedule: 5 Day Workweek (8 hours per day) or 4 Day Workweek (10 hours per day)

Location: Minneapolis

The Housing Case Manager provides housing case management services to residents living at Lydia Apartments in Minneapolis.

Housing Case Manager Duties:

• Establish supportive and motivational relationships with residents.
• Utilize person-centered, recovery-centered, harm reducation, and trauma-informed approaches with residents.
• Support residents in identifying achievable goals related to housing stabilization, tenancy skills, health and wellness, and employment.
• Support residents in building social relationships with other residents and community members.
• Enroll residents in available services that support them in reaching their goals.
• Connect clients to services and resources in the community.
• Maintain client records in electronic health record system (Credible).
• Partner closely with the Housing Case Manager Lead in building a welcoming environment for residents and a sense of community.
• Collaborate with the property manager, other service providers, staff members, consultants, family members, interns and/or volunteers.

Housing Case Manager Requirements:

• Valid driver's license, vehicle insurance, and acceptable driving record required
• Experience working with individuals who have experienced long-term homelessness is desired
• Experience working with individuals affected by mental health and/or chemical dependency issues is desired
