We are currently seeking a dedicated Medical Social Worker & Volunteer Coordinator (Hybrid Role) to join our interdisciplinary team. This unique position combines psychosocial support for patients and families with volunteer program management—ideal for someone passionate about both direct care and community engagement. We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. This position involves working closely with patients, families, and healthcare professionals to assess needs, develop treatment plans, and facilitate access to community resources.

Medical Social Worker Responsibilities

  • Conduct psychosocial assessments within 5 days of patient admission, identifying emotional, social, financial, and environmental needs.
  • Provide ongoing emotional and casework support to patients and families.
  • Participate in IDG and QAPI meetings; contribute to patient care planning.
  • Maintain accurate clinical documentation, including initial evaluations and progress notes.
  • Evaluate the effectiveness of interventions and participate in discharge planning as needed.
  • Educate and consult with the care team on psychosocial dynamics related to terminal illness.
  • Facilitate access to community resources and assist with bereavement and coping support.

Volunteer Coordinator Responsibilities

  • Oversee volunteer recruitment, training, and orientation programs.
  • Assign volunteers based on patient needs and ensure compliance with the Volunteer Plan of Care.
  • Supervise, evaluate, and support volunteers, including reviewing visit notes and coordinating ongoing education.
  • Maintain volunteer files and track monthly service statistics.
  • Serve as a liaison between volunteers and the IDG, ensuring smooth communication and coordination.
  • Participate in policy development for the volunteer program and assist with QAPI initiatives.

Qualifications

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • At least one year of healthcare experience (hospice experience preferred).
  • Valid driver’s license and insured vehicle.
  • Strong communication skills with the ability to work collaboratively within a multidisciplinary team.
  • Excellent organizational skills with attention to detail for accurate data collection and documentation.
  • Ability to manage time efficiently and multitask in a team-based environment.

Pay: $32.00 - $34.00 per hour
