About the Role – Health Care Coordinator

We are looking for a detail-oriented, compassionate, and personable Health Care Coordinator to be the welcoming voice of our practice and a vital part of our intake and care coordination team. The ideal candidate has strong customer service skills, is comfortable handling complex situations, and thrives in a fast-paced healthcare environment.

Key Responsibilities

• Conduct outbound and inbound calls with potential patients to educate them on our services and encourage enrollment
• Verify insurance coverage and benefits to determine patient eligibility
• Collect and accurately enter patient intake information into tracking systems
• Coordinate with therapists and the care team to ensure seamless service delivery
• Maintain high standards of data accuracy and integrity
• Perform ad hoc administrative tasks to support operations

What We’re Looking For

• Pleasant, clear, and professional phone manner with the ability to build trust and educate patients
• Excellent attention to detail with strong data entry skills
• Proficiency in Google Sheets, Excel, and G-Suite tools
• Solid business writing skills with correct grammar, punctuation, and spelling
• Prior homecare experience and knowledge of working with insurance companies in the home health field is a major plus
• A desire to be part of a growing organization that values transparency, collaboration, and operational excellence
