Role Description

This is a full-time hybrid role for a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC). The role has the option of remote work or on site. The responsibilities include providing therapy, conducting assessments, developing treatment plans, and offering case management services. The professional will also collaborate with a multidisciplinary team to ensure the highest level of patient care.

Qualifications

• Social Work and Medical Social Work skills
• Experience in Case Management
• Expertise in Mental Health and Behavioral Health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Master's degree in Social Work or Counseling
• Valid LCSW or LPC license in Connecticut (and NY preferred)
• Experience in a healthcare or mental health setting is a plus
