Requirements for the Job:

• Master Degree in Social Worker
• Minimum 1 year of Social Worker experience required
• Current ME state LCSW license Required
• Valid BLS Certifications
• Covid- Card

Roles and Responsibilities:

• Conduct comprehensive psychosocial assessments that identify residents' social, emotional, mental health needs and strengths.
• Collaborate with the interdisciplinary team (nurses, therapists, physicians, etc.) to develop and review individualized care plans addressing those needs.
• Advocate for residents' rights, dignity, independence, and psychological well-being.
• Provide therapeutic support and interventions to help residents adjust to and cope with transitions, behaviors, or mood issues.
