Job Description:Be a part of a world-class academic healthcare system at Ingalls Memorial Hospital as a Medical Social Worker.The Medical Social Worker provides clinical social work services through psychosocial assessments, treatment (counseling/social treatment interventions), and discharge planning services to patients and their families. Coordinates multi-disciplinary discharge planning process.Essential Job Functions

The Social Worker intervenes with patients who have complex social needs, require assistance with eligibility determination for social service programs and funding sources, and qualify for community assistance from a variety of special funds and agencies.

This role will receive referrals from individual care managers and inter-disciplinary team members (including physicians, staff nurses, and other members of the healthcare team).
Knows, follows, and actively supports the Philosophy, Mission, and Vision of Ingalls Hospital.
Adheres to all hospital policies, procedures, and standards of care.
Observes and adheres to all corporate compliance requirements and communicates issues as they arise with the appropriate parties as identified in the Corporate Compliance Plan.

Required Qualifications

Requires a current Illinois license as a Clinical Social Worker (LSW) or eligible and bachelor’s degree in social work from an accredited school of social work.
Minimum of one year's experience as a Social Worker.
Must have excellent verbal and written communication and organization skills.
Must obtain current and active BLS through the American Heart Association.
