- The Social Worker intervenes with patients who have complex social needs, require assistance with eligibility determination for social service programs and funding sources, and qualify for community assistance from a variety of special funds and agencies.
- offers crisis intervention to patients and families with psycho-social needs.
- This role will receive referrals from individual care managers and inter-disciplinary team members (including physicians, staff nurses, and other members of the healthcare team).

- Adheres to all hospital policies, procedures, and standards of care.
- Observes and adheres to all corporate compliance requirements and communicates issues as they arise with the appropriate parties as identified in the Corporate Compliance Plan.
- Requires a current Illinois license as a Clinical Social Worker (LSW) or eligible and bachelor’s degree in social work from an accredited school of social work.
- Minimum of one year's experience as a Social Worker.
- Must have excellent verbal and written communication and organization skills.
- Must obtain current and active BLS through the American Heart Association.
