Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for adults with severe mental illness (SMI) who are most at-risk of psychiatric crisis, hospitalization, and involvement with the criminal justice system. ACT is one of the oldest and most widely researched evidence-based practices in behavioral healthcare for people with severe mental illness.

ACT uses a multidisciplinary team approach with assertive outreach in the community. The team is comprised of a Supervisor, Team Lead, SUD Clinician, Vocational Clinician, Case Managers, Nurses, Peer Support Specialist and Administrative Assistant. The consistent, caring, person-centered relationships have a positive effect on outcomes and quality of life for the individuals that we serve. Research shows that ACT reduces hospitalization, increases housing stability, and improves quality of life for people with the most severe symptoms of mental illness.

Richmond Behavioral Health is seeking full-time ACT Housing Case Managers to perform intermediate professional work assessing client needs, developing, implementing and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting clients, maintaining records and files, preparing reports, and related work as apparent or assigned. Worked is performed under the supervision of the Program Supervisor.

Essential Functions

Assesses client service needs, capabilities, and appropriateness for services; presents options and services based upon a needs assessment.

Develops an individual service plan of care and services appropriate to the evaluation with the client, reviews and updates service plans.

Makes referrals and linkages to appropriate agencies for services, coordinates client services and treatment with multiple service providers and agencies.

Complete thorough housing barrier assessment and housing plan for each program participant.

Assist program participants with process of obtaining appropriate housing subsidy.

Assist program participants with locating and securing permanent housing. Act as liaison between program participants, landlords, and staff.

Actively outreach and recruit landlords and provide appropriate information about Authority programs and services.

Conduct initial eligibility and annual/interim re-certifications (Housing Quality Standards (HQS) inspections and follow ups and financial updates)

Monitor and facilitate moves/relocation of program participants. Recalculate rents when necessary (either due to income change, recertification, or change in rent amount)

Evaluates the quality of services provided and changes in client's condition and counsels clients as necessary; evaluates client's environment for safety, security, negative factors and productivity; compiles and analyzes data relating to complaints; identifies and works to resolve problems.

Plans and facilitates group sessions with clients participating in community activities and/or to promote recovery and educate clients.

Provides motivational counseling to clients to facilitate/improve engagement in services that support recovery. Assists clients with activities of daily living including hygiene/bathing.

Assists with the orientation and training of newly hired staff;

Orients and trains newly hired staff; evaluates staff performance as directed.

Serves as liaison to public agencies and provides information regarding Authority programs and services.

Attends inter-disciplinary team meetings to discuss decisions for client plan of care; provides or arranges transportation; assists with discharge planning.

Provides crisis intervention and prevention services.

Prepares a variety of reports; prepares and maintains client charts and other records.

Attends meetings, staffings and conferences as they relate to client, staff and program needs. Attends court hearings; testifies in court.

Assists in curriculum development and resource tools.

Position Requirements

General knowledge of community and agency resources and programs of regional and state sources for the community service population; general knowledge of interviewing and supportive counseling techniques; general knowledge of social casework techniques; general knowledge of human development and behavior; general knowledge of the theories, principles and techniques of individual, family and group therapy; ability to solve problems within scope of responsibility; ability to analyze facts and exercise sound judgment in arriving at conclusions; ability to communicate complex ideas effectively, orally and in writing; ability to prepare clear and concise reports; ability to establish and maintain effective working relationships with clients, medical professionals community partners, agencies, associates and the general public.

Education and Experience

Minimum of a bachelor's degree in any field required; current enrollment may be considered for those professionals who are registered as a QMHP-T with the Virginia Board of Counseling.

Must be registered as a QMHP or QMHP-T with the Virginia Board of Counseling or hold a graduate degree and meet licensure eligibility. (Please refer to the Board of Counseling website to register and for additional information).

Experience with individuals with mental health, behavioral challenges, substance use disorders and intellectual/developmental disabilities is essential.

Master's degree preferred.

One year of experience working with individuals with serious mental illness or intellectual disabilities in a behavioral health setting.

Qualifying case management staff will be eligible for a $3K sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.

Special Requirement

Applicants must have a valid Virginia driver's license.

Training in verbal de-escalation and non-violent crisis intervention (or obtained during orientation).

Starting Salary Range
$50,899.06
