PathWell Health, a prominent provider in both home healthcare and hospice services, is looking for an experienced and driven Community Liaison to expand our reach in Loudoun County and parts of western Fairfax County, Virginia. This role demands a deep understanding hospice care, including eligibility criteria and the healthcare setting, ideally from a clinician's perspective. The Community Liaison will be responsible for building and nurturing relationships with facilities such as Assisted Living Facilities (ALFs), Skilled Nursing Facilities (SNFs), doctors' offices, and hospitals-specifically with case managers, Executive Directors, and discharge planners-to secure referrals for our services.

This role is pivotal in strengthening PathWell Health's presence and impact within the hospice landscape, ensuring we remain a preferred choice for comprehensive patient care in the communities we serve.
Responsibilities:
- Design and execute marketing strategies to promote PathWell Health's Hospice services to targeted facilities and referral sources.
- Forge and maintain relationships with key stakeholders in ALFs, SNFs, Community Organizations, Senior Centers, LTCFs, and medical practices, particularly those specializing in oncology, cardiology, pulmonology, and family/internal medicine with a geriatrics focus.
- Present the benefits and value of PathWell Health's comprehensive care services through persuasive sales presentations.
- Engage with medical professionals to inform and educate about our unique hospice offerings.
- Drive referral generation by identifying, qualifying, and following up on leads from various sources to maintain a strong pipeline of potential patients.
- Manage and update all sales activities and contact information accurately in CRM software.
Qualifications:
- Demonstrated success in healthcare sales, with a preference for experience in hospice services.
- Solid understanding of healthcare industry standards, especially regarding hospice care regulations and services.
- Exceptional communication, presentation, and negotiation skills, coupled with strong networking and relationship-building capabilities.
- Independent and motivated, capable of working both autonomously and as part of a team.
Preferred Qualifications:
- Clinical background (e.g., nursing or medical social work, provides a strong foundation for understanding patient needs and care requirements.
- Experience in developing business with ALFs, healthcare facilities, and referral sources.
- Deep knowledge of the local healthcare environment and key players within the community.
- A keen insight into geriatric care and the medical specialties associated with aging populations.
