LSA Recovery, Inc. is an 822 Medically Supervised Outpatient Chemical Dependency Program, which provides a variety of services to individuals as well as family members seeking help with substance abuse and chemical dependence problems.
We believe that chemical dependence treatment requires a detailed knowledge of the history and personality of each patient. This is accomplished by professional and knowledgeable staff that will conduct a complete and thorough interview with the patient and his/her family, as well as contact with their primary care physician. The data is then evaluated by various professional staff members in order to develop a personalized treatment plan. We organize the plan of care to enable the patient to proceed through the recovery process with a support network, increase their self-esteem, and understand the effects of chemical dependence.
Position :* Counselor , Outpatient Chemical Dependency Program.*
Report to : Director / Clinical Supervisor
Purpose : To provide all the basic therapeutic services required by patients during their treatment and aftercare experience. The counselor is expected to act as a catalyst in the therapeutic process of the patient's treatment episode.
_**Responsibilities include:_*
- Provide intake evaluations, pre-admission assessments, and brief screenings. Define appropriate level of care based
- Obtain initial authorization for all relevant cases. Facilitate new patient’s entry into treatment (clinic rules, procedures) and coordinate process with the medical, nursing and administrative staff. Request patient records from previous treatment facilities where indicated.
- Conduct observed urines and breathalyzers for patients
- Monitor patient attendance and toxicology results on an ongoing basis. Evaluate patients’ use of illicit substances, alcohol and prescribed medication
- Assume a patient caseload and is responsible to maintain patients’ records in substantial OASAS compliance
- Formulate individualized treatment plans for patient caseload that is in OASAS compliance
- Conduct individual, group and family psychotherapy as well as case management, crisis intervention and referral services.
- Assess patient needs and refer to supplementary services such as Self Help, Mental Health, Educational programs, Vocational training, Employment services, Social Services, Legal or Family Planning.
- Develop progress notes for every face-to-face contact on same day of service including Evidenced Based Interventions.
- Record all telephonic conversations related to patients in the case record on real time.
- Participate in multidisciplinary case conferences and team meetings.
- Maintain the security and confidentiality of the patient and the patient’s medical record at all times.
- Attend conferences, seminary, lectures, webinars, etc… as considered appropriate by the Clinical Director.
- Perform other job related duties as required.
Requirements
Active NYS license required: CASAC-T, CASAC Level #2 ,LMSW, LCSW, LMHC, LMFT
