About Job

Bishop Noa Home is currently seeking a full-time Social Worker for our Skilled Nursing Facility. This position participates in resident care management and ensures that needs are met through Social Service programming and appropriate referrals to outside agencies and community resources.

The ideal candidate will have prior long-term care experience, excellent communication skills and a compassionate and empathetic demeanor.

· Conduct social service assessments and collect social histories.

· Identify and provide for each resident’s social, emotional and psychological needs.

· Identify and seek ways to support residents’ individual needs and preferences, customary routines, concerns and choices.

· Coordinate and facilitate resident care conferences including initial, quarterly, change of condition, special needs, and discharge conferences

· Coordinate discharge planning services.

· Develop and maintain individualized care plans

· Participate in the planning of the resident admissions, transfers, and discharges.

· Complete all necessary documentation and screenings according to federal, state, local, and facility policies and procedures.

· Participate in team meetings, care conferences, and MDS data completion and collection.

· Serve on the Quality Assurance Committee and other committees as requested.

· Act as a facility resource to provide intervention with difficult resident and family issues, utilizing grievance and conflict resolution skills.

· Assist residents/responsible parties with financial and legal matters such as guardianship/conservatorship

  • Licensed Social Work certificate, current and valid; BSW preferred.
  • Knowledge of and ability to obtain community resources
  • Demonstrated excellent oral and written communication skills.
  • MDS and Short Term Discharge planning experience required
  • Minimum of one (1) year experience in case work and/or working with an elderly population.(Preferred)

Pay: $44,170.45 - $53,194.52 per year
