⏰ Schedule: Monday–Friday, 9 am–5 pm (35 hours/week)

???? Travel Pay: $1,895/week ($17/hr W2 + $1,300 weekly per diem)

???? Local Pay: $1,645/week

About the Opportunity:

We’re seeking a passionate and dedicated Licensed Master Social Worker (LMSW) for an impactful hybrid contract role based in New York City. This position focuses on providing care management, health coaching, and coordination for members, including those who are HIV-positive, transgender, or experiencing homelessness. What You’ll Do:

✅ Deliver telephonic and in-person care management, transition of care, and follow-up

✅ Create individualized Plans of Care, collaborating with members, families, and providers

✅ Support access to comprehensive healthcare services and resources

✅ Educate and coach members on HIV disease, risk reduction, and chronic conditions like hepatitis C, diabetes, and hypertension

✅ Communicate regularly with providers and case managers to ensure coordinated care

✅ Document goals, interventions, and progress in electronic health records

✅ Mentor and support team members and participate in case conferences

✅ Engage in special projects, audits, quality improvement, and community health initiatives

Who We’re Looking For:

✔ LMSW license issued by NYSED (Primary Source Verification required)

✔ Minimum 1 year of LMSW experience

✔ Strong communication, documentation, and organizational skills

✔ Commitment to promoting health equity and serving diverse, underserved populations
