TRAVELING REQUIRED- IN PERSON VISITS
Function: The mission of Lydia Home Association is to care for children and strengthen families. The residential program at Lydia Home Association serves youth ages 6-17, who are involved with the Department of Child and Family Services (DCFS). The youth reside at Lydia Home and are provided with a structured living environment, mental health treatment, wrap around services, and case management support.
Job Description: The After-Care Case Manager is responsible for providing case management and clinical support to youth and their families for six months after discharge from residential trauma treatment program. Supports will include phone calls, in person visits, family therapy/parent coaching sessions, all in collaboration with the youths' residential treatment team. Post discharge staffings; overseeing and tracking weekly communication and visits with discharged clients. Collaboration with triads. Run a weekly group with kids who have an identified placement and are clinically ready or within a few months
Requirements
Minimum Requirements: Masters level person who would be qualified to provide family therapy - eligible for licensing in Social Work or Psychology. Job requires some night and weekend hours (flexibility for client group and parent events). Willing to travel - will do in person visits (can use agency vehicle). Must have good writing and social skills. The case manager must have an interest and concern for children and their families. It is also necessary to be able to communicate and work with all members of the child care team. Must be willing to support the mission of Lydia Home Association (LHA).
