Role Description

This is a full-time, on-site role for a Social Worker located in New Haven, CT. The Social Worker will be responsible for providing emotional support and guidance to patients and their families, coordinating with medical staff for patient care planning, conducting assessments and developing care plans, and facilitating discharge planning. The Social Worker will also address social, psychological, and emotional needs to improve patients' overall well-being.
