GentleCare Therapy, a thriving pediatric developmental therapy practice, is seeking a highly proficient and experienced Care Coordinator for our ABA program in NJ. Responsibilities:
As a Care Coordinator, you will be integral to the ongoing client services, ensuring seamless operations and outstanding client and staff satisfaction. Your responsibilities will include:
- Client Services Management: Oversee all aspects of ongoing client services, including scheduling, managing call-outs, and ensuring efficient restaffing.
- Optimized Scheduling: Develop and maintain clinicians' and clients' schedules, ensuring appropriate client-to-therapist ratios. You will be the primary scheduling point of contact for both clients and staff.
- Process Enhancement: Direct and optimize client scheduling processes to maximize therapist utilization and client access to services. This includes monitoring scheduling metrics, identifying trends, and implementing strategies to improve efficiency and reduce wait times.
- Problem Solving: Address complex scheduling challenges and client requests with a proactive and solution-oriented approach.
- Assessment Coordination: Ensure the timely completion of both initial assessments and reassessments.
- Integrated Care Coordination: Facilitate care for clients receiving services from multiple disciplines and coordinate substitute clinicians as needed to maintain continuity of care.
- Effective Communication: Answer phone calls and respond to inquiries professionally and promptly. Maintain ongoing email and phone communication with families and staff, fostering strong relationships.
- Interdepartmental Collaboration: Collaborate with Human Resources to ensure sufficient staffing levels for new client intake.
- Record Management: Accurately maintain and regularly audit client records to ensure data integrity.
- Authorization Oversight: Manage and meticulously track client authorizations to ensure uninterrupted service delivery.
Qualifications:
We are seeking a candidate who embodies professionalism, efficiency, and a genuine passion for making a difference. Required qualifications include:
- Extensive Medical Office Experience: A minimum of 5 years of Care Coordinator/Case Manegment expernce is required.
- Exceptional Communication Skills: Demonstrated excellence in written and verbal communication, with impeccable phone and email etiquette.
- Key Characteristics: Professional demeanor, strong ethical standards, adaptability, exceptional organizational skills, meticulous attention to detail, and a consistently positive and compassionate attitude.
- Technological Proficiency: Highly proficient with various technologies, including Microsoft Excel, Google Workspace, Electronic Medical Records (EMR) systems, and Central Reach.
Job Type: Full-time
Pay: $55,000.00 - $65,000.00 per year
