About Job

RiseWell Community Services is seeking a Part Time Respite Plus Worker reporting to the Program Supervisor within the Outreach Program in Patchogue. Schedule: Part time, up to 19 hours per week

Our Outreach programs provide an array of services designed to engage individuals who are reluctant to seek help or experience barriers to access. Services are brought to the person where they are located, whether that be at home, the street, a hospital, or another location. As a part of this team, you’ll assist those we serve to have their basic needs met by providing referrals, linkage, and hands-on assistance with obtaining food, employment, housing, and financial assistance.

QUALIFICATIONS:

High School Diploma. Experience in the Mental Health field. Good communication skills. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

JOB FUNCTION:

Provide Respite care to individuals with mental illness in order to increase their independent living skills and to provide time off for family members or caretakers.

ESSENTIAL FUNCTIONS:

A. Provide Respite care to individuals with mental illness.

B. Interact with individuals on a one and one basis providing support including advocacy, outreach, companionship, and education.

C. Accompany individuals to social and community activities.

D. Maintain proper documentation.

E. Attend staff meetings, in-service training and workshops as scheduled.

F. Provides peer support to others.

GPerform other related duties as needed.
