Nurse Manager - Trinity Health Saint Joseph PACE


Position Purpose


Trinity Health PACE is seeking a compassionate and driven Nurse Manager to coordinate and supervise nursing services provided to our participants, either in our Day Center or in their homes. This leadership role ensures high-quality care delivery within the PACE model for frail and elderly adults.


What You Will Do


  • Lead and manage nursing services to meet participant needs




  • Supervise and mentor clinical staff, ensuring compliance and high standards




  • Collaborate with the interdisciplinary team (IDT) for care planning and coordination




  • Maintain adherence to federal, state, and organizational regulations




  • Oversee infection control, budget management, and quality improvement initiatives




  • Engage with participants and families to support care and services




Minimum Qualifications


  • Current RN license in the state of practice




  • BSN required; active CPR certification




  • At least 3 years of healthcare experience, including geriatrics




  • Minimum 1 year of experience with a frail or elderly population or willingness to train




  • Previous supervisory experience required




  • Valid driver's license and proof of insurance
