Nurse Manager - Trinity Health Saint Joseph PACE
Position Purpose
Trinity Health PACE is seeking a compassionate and driven Nurse Manager to coordinate and supervise nursing services provided to our participants, either in our Day Center or in their homes. This leadership role ensures high-quality care delivery within the PACE model for frail and elderly adults.
What You Will Do

Lead and manage nursing services to meet participant needs

Supervise and mentor clinical staff, ensuring compliance and high standards

Collaborate with the interdisciplinary team (IDT) for care planning and coordination
-
Maintain adherence to federal, state, and organizational regulations
-
Oversee infection control, budget management, and quality improvement initiatives

Engage with participants and families to support care and services
Minimum Qualifications
-
Current RN license in the state of practice
-
BSN required; active CPR certification
-
At least 3 years of healthcare experience, including geriatrics
-
Minimum 1 year of experience with a frail or elderly population or willingness to train
-
Previous supervisory experience required

Valid driver's license and proof of insurance
