Job Title: Licensed Master Social Worker (LMSW) - Hybrid
Position: Full-Time

Location: Bronx, NY

Job Summary:

The Licensed Master Social Worker (LMSW) will be responsible for performing daily social work operations, including assessments, treatment planning, and coordination of care. This role supports integrated behavioral health services in collaboration with a multidisciplinary team.

Key Responsibilities:

• Conduct assessments of patients referred by Primary Care Providers (PCPs) using evidence-based tools.
• Provide treatment for behavioral health conditions using approaches such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, crisis intervention, Interpersonal Psychotherapy, and behavioral activation.
• Participate in collaborative care team meetings and make treatment recommendations.
• Monitor patient progress through care planning and coordination with clinical teams and health homes.
• Deliver crisis intervention services and perform risk assessments as needed.
• Engage clients with limited behavioral health service utilization through outreach.
• Maintain timely and accurate documentation of services.
• Participate in weekly clinical supervision.
• Attend team meetings and case conferences as required.

Qualifications:

• Active LMSW Licensure.
• Master’s degree in social work from an accredited institution.
• Proficiency in using behavioral health screening tools and conducting biopsychosocial assessments.
• Ability to diagnose and treat behavioral health conditions using DSM-V criteria.
• Experience in risk assessment and collaboration with psychiatric and clinical teams.
• Competency in using electronic health records (EHR) for documentation and data review.
• Strong communication skills to effectively convey clinical information to integrated care teams, including PCPs, NPs/MDs, and Health Home Care Management staff.
• Minimum of 1 year experience required.

This is a hybrid position that requires on-site visits only 2 to 3 times a month, and only when necessary
