Description:

Summary:

The License Clinical Social Worker will provide services to children, adults, and families and/or groups. The clinician will develop plans with the client and implement therapeutic treatment plan in a strength-based model in the doctors’ offices. The licensed clinician will provide assessments, referrals and follow-up appointments. This position addresses goal management and progress tailored to patient health needs; provides psychosocial assessments, discharge planning, patient consultation regarding illness, diagnosis, treatment, and/or life situations.

Essential Responsibilities

  • Provides psycho-social assessments of patients to identify emotional, social, and environmental strengths and/or problems related to their diagnosis, illness, treatment, and/or life situation
  • Provides patients with knowledge regarding the patient's diagnosis, illness and/or treatment
  • Familiarity with hospital, medical center and/or health system resources to access and provide for patient care needs.
  • Participate in case review meetings.
  • Documents assessments plans, interactions, and interventions according to WMCHC/health system guidelines and standards.
  • Remain current with social work and health care developments
  • Plan, develop, and facilitate educational activities for patients including but not limited to presentations, workshops, goal-setting and management, addressing techniques for health improvement, trouble areas, etc.
  • Stays in compliance with all HIPAA regulations

Position Requirements

Qualifications

  • Bilingual (English/Spanish) required
  • Master’s Degree in Social Work, Psychology or related field.
  • Current CA clinical license (LCSW).
  • Minimum of two years in clinical experience post master’s degree, preferably working in a community clinic setting or a mental health agency that serves a wide variety of clients.
  • Exceptional interpersonal, written, and oral communication skills
  • Able to work independently and in a team; self-directed.
