Position Summary:

The Resident Care Coordinator plays a vital role in ensuring quality care for our residents by coordinating health services, supporting the caregiving team, and communicating with families and healthcare professionals. This leadership position bridges clinical care with compassionate service.

Key Responsibilities:

• Coordinate and oversee resident care plans in collaboration with nurses and caregivers
• Monitor changes in resident health and ensure appropriate interventions
• Communicate regularly with families, physicians, and healthcare providers
• Conduct new resident assessments and assist with move-in transitions
• Train, support, and supervise care staff to ensure compliance and excellence in care
• Maintain accurate documentation and participate in audits or state inspections
• Participate in care planning meetings and contribute to community-wide wellness initiatives

Qualifications:

• 2+ years of experience as a Resident Care Coordinator in senior living, long-term care, or healthcare
• Strong leadership, organizational, and communication skills
• Outstanding computer and software management skills.
• Familiarity with electronic medical records and state compliance requirements
• Passion for working with older adults and supporting families
