Provides general medical social work services to the Agency’s patient population, facilitates communication between patients and staff, establishes and/or facilitates cooperation and services between the Agency and the community at large, explore the potential for new services and assist with the development of expanded services. Responsibilities
Provides services to the patient and family to resolve social or emotional problems that could impede the effective treatment of the patient’s medical condition.
Identifies the social and emotional factors related to the patient’s illness, need for care, response to treatment and adjustment to care.
Assesses the relationship of the patient’s medical and nursing requirements to the patient’s home situation, financial resources and availability of community resources.
Takes appropriate action to obtain available community resources to assist in resolving the patient’s problem.
Provides counseling services, as appropriate when required by the patient.
Provides services to the patient’s family member or caregiver on a short-term basis when the HHA can demonstrate that a brief intervention is necessary to remove a clear and direct impediment of the effective treatment of the patient’s medical condition or to his/her rate of recovery.
Participates in patient care conferences as needed.
Performs within their recognized scope of practice.
Assist in staff development on a continuing basis
Supervises BSW as needed.
Attends in-services and department staff meetings as required.
Work with supervisor to find solutions and resolve work-related problems including: procedures, peer relationships and management directives.
Qualifications, Knowledge, Skills & Abilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
M.S.W. from an accredited School of Social Work.
Experience in Medical Social Work. One (1) year of Home Health experience preferred.
Skills in problem solving, individual and group counseling, ability to relate effectively with patients in crisis.
Ability to provide own transportation.
