Community Health Worker (CHW) - County Public Health Department
Location: Oxnard, CA - Public Health Building
Schedule: Monday-Friday, 8:00 AM-5:00 PM (flexible start time between 7:30-9:00 AM; 30 or 60 min lunch)
Language Requirement: Bilingual in English and Spanish (read, write, and speak fluently)

Position Overview:
We are seeking a compassionate and detail-oriented Community Health Worker (CHW) to join a County Public Health intake team in Oxnard, CA. This CHW will help support vulnerable populations by guiding families through the eligibility and intake process for public health services, with a focus on Medi-Cal access and medical program coordination.
You will work closely with nurses, families, and program stakeholders to ensure timely, accurate case processing, documentation, and communication. This position is primarily office-based, with occasional participation in local public health events.
Key Responsibilities:
- Serve as part of the intake team, processing an average of 300 referrals monthly
- Review incoming referrals to determine initial medical eligibility
- Support families through the Medi-Cal application and verification process
- Review nurse case notes and coordinate interviews with families
- Open new cases once residential and financial criteria are confirmed
- Conduct introductory calls with families, explaining program services and expectations
- Collect and document detailed demographic and eligibility data
- Issue service authorizations based on nurse directives
- Handle mailing and clerical processing of program documents
- Meet with families either in person at the Oxnard office or virtually
- Provide back-up case management coverage when other CHWs are out
- Occasionally represent the program at local events (e.g., in Camarillo)
Qualifications:
Must-Have:
- Bilingual (English/Spanish) - fluent in speaking, reading, and writing
- Prior experience as a Community Health Worker or Case Manager
- Experience working with Medi-Cal programs and navigating public health benefits
- Familiarity with EMR systems (e.g., Callfax, Filenet, Cerner)
- Strong data entry and documentation skills
- Excellent communication and interpersonal skills
Nice-to-Have:
- Experience in a public health or government setting
- Knowledge of social determinants of health and resource navigation
- Prior work with diverse or underserved populations
Work Environment & Team:
You'll join a team of 10 CHWs and one Nurse, supporting a busy but collaborative intake unit. The team works off an organized queue, with caseloads split alphabetically. This role is office-based with potential coverage responsibilities across the department.
