Position Overview:
A healthcare organization in Upland, CA is seeking a Licensed Clinical Social Worker (LCSW) to provide high-quality behavioral health services in an outpatient clinic setting. The LCSW will conduct assessments, develop treatment plans, provide therapy, and collaborate with an interdisciplinary team to support adult patients experiencing a range of mental health and substance use challenges.

Key Responsibilities:

• Provide individual and group counseling to adult clients
• Conduct biopsychosocial assessments and develop evidence-based treatment plans
• Facilitate therapy for individuals enrolled in substance use recovery programs, including court-ordered services (Prop 36)
• Document services accurately in the Electronic Health Record (EHR)
• Coordinate care with internal team members and external community partners
• Perform Medi-Cal eligibility verifications and assist with discharge planning
• Participate in staff meetings and clinical training
• Maintain confidentiality and comply with HIPAA and professional ethical standards

Patient Population:

• Adults with behavioral health needs, including mental illness and substance use disorders
• Individuals enrolled in outpatient or day health programs
• Underserved and low-income community members

Qualifications:

• Current LCSW license in California (required)
• Master’s degree in Social Work (required)
• Minimum two years of post-master’s clinical experience in a healthcare or behavioral health setting
• Experience with severely mentally ill (SMI) adults and substance use recovery preferred
• Strong communication, documentation, and clinical assessment skills
• Bilingual Spanish/English preferred
• Valid CA Driver’s License and reliable transportation
