INTRODUCTION:
Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US Health Care System requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
∙ Provides psycho-social assessments of patients to identify emotional, social, and environmental strengths and/or problems related to their diagnosis, illness, treatment, and/or life situation
∙ Provides patients with knowledge regarding the patient's diagnosis, illness and/or treatment
∙ Familiarity with hospital, medical center and/or health system resources to access and provide for patient care needs
∙ Participate in case review meetings
∙ Documents assessments plans, interactions, and interventions according to CNHF system guidelines and standards.
∙ Plan, develop, and facilitate educational activities for patients including but not limited to presentations, workshops, goal-setting and management, addressing techniques for health improvement, trouble areas, etc.
∙ Remain current with social work and health care developments.
∙ Stays in compliance with all HIPAA regulations
∙ Performs other duties as assigned.

EDUCATION and/or EXPERIENCE
∙ Collaborative and team oriented
∙ Demonstrate strong problem-solving, communication, and leadership skills ∙ Excellent organizational skills, ability to manage multiple tasks and meticulous attention to detail
∙ Possess compassion and empathy
∙ Computer literacy required (Microsoft Office Suite)
∙ Willing to travel to support clinical and foundation activities
∙ Understanding of cultural issues, community resources, and family dynamic ∙ Ability to communicate well with people of diverse cultural professional and experiential backgrounds
∙ Knowledge of laws and ethical standards pertaining to clinical care
∙ Master’s Degree in Social Work
∙ Current CA clinical license (ACSW)
∙ Minimum of two years in clinical experience post master’s degree, preferably working in a community clinic setting or a mental health agency that serves a wide variety of clients
∙ Exceptional interpersonal, written, and oral communication skills
∙ Able to work independently and in a team; self-directed
∙ Bi-lingual (English/Spanish) preferred, not required
