About The Position
Job Title: Social Worker/RN Case Manager
Profession: Nursing
Specialty: LCSW
Duration: 13 months
Shift: Monday to Friday
Hours per Shift: 8am - 5pm, Weekend on-call rotation (1 weekend per month)
Experience: Minimum of 3 years
License: Valid, current, full and unrestricted Professional License to practice as LMSW or LCSW in any state of the United States or territories.
Certifications: Must have and maintain current Basic Life Support (BLS) certification throughout employment.
Must-Have: Experience and knowledge of discharge planning, case management, or utilization review processes.
Must have 1 year of traveling experience as a contractor.
Demonstrate knowledge of electronic health record systems.
Description:
The incumbent serves as an Outpatient/Inpatient Social Worker/RN Case Manager.
The individual is responsible for direct supervision and supportive contact for identified high-risk patients.
The Social Worker/RN Case Manager designs and manages a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency.
Responsibilities include assessment, service planning, resource acquisition, monitoring progress, and responding to emerging client needs.
The role involves linking clients with social service providers, health care providers, substance abuse, and mental health providers to achieve specific goals.
Utilization review of patient cases in inpatient and outpatient service departments is a key responsibility.
The incumbent will review the medical necessity of referrals to specialty providers for services not normally provided.
Collaboration with a multidisciplinary team is essential to promote the efficient and effective use of healthcare resources.
Goals include providing quality care, decreasing fragmentation of care, enhancing patient self-care and quality of life, and ensuring cost containment.
The incumbent will work closely with healthcare providers to implement appropriate resources in fulfillment of utilization goals.
The Social Worker/RN Case Manager reports to the Director of Case Management/Care Coordination.
