Responsibilities

The Community Liaison, Home Health, and Hospice plays a vital role in communication and business development within our varying service sectors. This position acts as the main point of contact for our community of referral partnerships and will explain the benefits of our programs to patients, caregivers, powers of attorney, and guardians.

Key Duties:

  • Conduct market analysis and assess the competitive landscape to effectively position our healthcare services.
  • Achieve goals for face-to-face and virtual meetings with key decision-makers.
  • Understand the eligibility and enrollment processes for our range of services.
  • Track performance in CRM to adhere to budgeted targets for the region.
  • Engage directly with patients, caregivers, and guardians to assess eligibility for our programs.
  • Guide on benefit enrollment program processes.
  • Secure Voluntary Alignment Forms from patients and caregivers.
  • Respond to inbound queries regarding our programs with professionalism.
  • Triage patient concerns accurately to the relevant department.
  • Document and oversee outreach and scheduled meetings with external contacts.
  • Ensure that meetings with patients and caregivers are timely and productive.
  • Follow company policies for patient outreach and comply with PHI and HIPAA guidelines.
  • Maintain open lines of communication with management and local support staff.
  • Perform additional duties as necessary.

Qualifications

Required Skills and Experience:

  • High school diploma or GED
  • At least six months of experience in office administration or outside sales
  • A valid driver's license and a good driving record are required
  • Exceptional written and verbal communication skills
  • The ability to work independently and manage multiple tasks efficiently
  • Strong organizational abilities
  • Proficient in Microsoft Office applications and reporting software
  • Proven ability to cultivate relationships with eligible patients over the phone

Preferred Skills:

  • Self-motivated individual who thrives with little supervision after training

Pay Transparency

Compensation varies based on individual qualifications, experience, and other related factors.
