We are seeking a dedicated and skilled Psychiatry/Mental Health Nurse Practitioner or Physician Assistant to join our healthcare team. The ideal candidate will provide high-quality patient care while rounding Psychiatry/Mental Health patients at Long-Term Care, Skilled Nursing, and Assisted-Living facilities via in-person and or telemedicine modalities.
The Nurse Practitioner/Physician Assistant will perform regular medication management and reconciliation for patients in these settings.
Collaboration is a key component of this NP/PA’s role, requiring effective communication with other healthcare providers, patients, and families to ensure cohesive and continuous care. Their expertise and compassionate care are vital to the well-being of individuals struggling with mental health challenges.
Duties
- Conduct thorough patient assessments, including medical history.
- Develop and implement individualized care plans based on patient needs.
- Monitor and evaluate patient progress, adjusting treatment plans as necessary.
- Provide education to patients and families regarding their mental health.
- Maintain accurate medical records and documentation in compliance with regulations.
- Collaborate with other healthcare providers, patients, and families to ensure cohesive and continuous care.
- Provide compassionate and expertise care to patients.
- Stay updated on the latest advancements in healthcare practices, particularly in areas such as Psychiatry/mental Health.
Requirements
- Valid Nurse Practitioner/Physician Assistant license for Psychiatry/Mental Health in the State of California.
- DEA License.
- BLS
- Proficiency in medical terminology & EMR.
- Strong skills in patient assessment and critical thinking.
- Excellent communication skills for effective interaction with patients, team members, and families.

Job Type: Full-time
Pay: $130,000.00 - $160,000.00 per year
