About Job

We are seeking a dedicated and skilled Psychiatry/Mental Health Nurse Practitioner or Physician Assistant to join our healthcare team. The ideal candidate will provide high-quality patient care while rounding Psychiatry/Mental Health patients at Long-Term Care, Skilled Nursing, and Assisted-Living facilities via in-person and or telemedicine modalities.

The Nurse Practitioner/Physician Assistant will perform regular medication management and reconciliation for patients in these settings.

Collaboration is a key component of this NP/PA’s role, requiring effective communication with other healthcare providers, patients, and families to ensure cohesive and continuous care. Their expertise and compassionate care are vital to the well-being of individuals struggling with mental health challenges.

Duties

  • Conduct thorough patient assessments, including medical history.
  • Develop and implement individualized care plans based on patient needs.
  • Monitor and evaluate patient progress, adjusting treatment plans as necessary.
  • Provide education to patients and families regarding their mental health.
  • Maintain accurate medical records and documentation in compliance with regulations.
  • Collaborate with other healthcare providers, patients, and families to ensure cohesive and continuous care.
  • Provide compassionate and expertise care to patients.
  • Stay updated on the latest advancements in healthcare practices, particularly in areas such as Psychiatry/mental Health.

Requirements

  • Valid Nurse Practitioner/Physician Assistant license for Psychiatry/Mental Health in the State of California.
  • DEA License.
  • BLS
  • Proficiency in medical terminology & EMR.
  • Strong skills in patient assessment and critical thinking.
  • Excellent communication skills for effective interaction with patients, team members, and families.
