JOB SUMMARY

The Reentry Facilitator/Care Manager reports to the Assistant Director of Workforce & Career Pathways Development and is responsible for supporting participants referred by County Probation. This includes building and maintaining effective working relationships with probation officers and a network of local service providers. The role involves conducting pre-release screenings, facilitating evidence-based Cognitive Behavioral Intervention (CBI) classes, tracking client progress, and documenting individualized case notes and program outcomes.

This position is full-time temporary, scheduled to run through September 2025.

JOB DUTIES AND RESPONSIBILITIES

  • Work directly with probationers, the County Probation and Sheriff’s Departments, and local service providers to plan community reintegration following incarceration
  • Conduct initial pre-release screening to determine the level of service and support needed with a focus on education, employment, family, recovery, and community
  • Upon hire, successfully complete certifications in CBI-Employment and Motivational Interviewing
  • Develop individualized reentry and stabilization plans with assigned participants
  • Facilitate daily workshops for probationers as part of the ten-week, cohort-based CBI-Employment program
  • Develop, revise, and implement curriculum that aligns with the evidence-based practices of the CBI program
  • Develop and facilitate strategy for community education and awareness of the needs of formerly incarcerated participants
  • Network with other reentry programs to develop and maintain a list of resources for services referrals
  • As needed, refer participants to probation department employment programs
  • Maintain accurate, up-to-date records on participants, employers, placements, and related data using Apricot 360 or other approved client databases, in compliance with program standards
  • Provide ongoing reports of progress, outcomes, and activities
  • Conduct intake and exit interviews to assess client needs, progress, and program outcomes
  • Monitor and evaluate participant goal achievement progress related to their Personal Responsibility Plan
  • Maintain timely and accurate documentation of participants progress and maintains appropriate documentation in case files
  • As needed, assist with program orientation sessions
  • Coordinate efforts the of internal and external partners to improve case management services to participants
  • Provide counseling and crisis intervention services to program participants
  • As needed, take a lead role in the planning and implementation criminal justice, reentry training and other initiatives
  • Other duties as assigned

KNOWLEDGE AND SKILLS

  • Excellent public relations, community organizing, negotiation, mediation, conflict management, and meeting facilitation skills
  • Knowledge of reentry services, probation systems, and community-based support programs
  • Understanding of Cognitive Behavioral Interventions (CBI) and their application in workforce development and behavior change
  • Skilled in motivational interviewing, trauma-informed care, and client-centered case management
  • Ability to conduct intake, assessment, and discharge planning with accuracy and sensitivity
  • Strong interpersonal and communication skills, with the ability to build rapport across diverse populations
  • Proficient in using case management software, such as Apricot 360, for accurate documentation and data tracking
  • Ability to facilitate group workshops and trainings, particularly within cohort-based programming
  • Strong problem-solving and crisis intervention skills
  • Advanced proficiency in Zoom, Microsoft Office, and Google Workspace
  • Able to maintain strict confidentiality and take all precautions when handling sensitive information
  • Ability to effectively and efficiently work from home and independently, with high levels of self-accountability
  • Must have strong administrative and organizational skills including budget development and management, and grant administration and reporting

EDUCATION AND WORK EXPERIENCE

  • BA degree (MA degree highly preferred) in Psychology, Sociology, Social Work, Counseling or a closely related field is preferred
  • 2 years of case management, counseling, and assessment experience
  • Previous reentry experience preferred OR Equivalent combination of education and experience

PHYSICAL REQUIREMENTS

  • Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed
  • Frequent talking and hearing conversations
  • Frequent and prolonged standing, walking, and sitting
  • Long periods of walking, sitting at a desk and/or computer station
  • Lifting items that weigh up to 25 lbs. or more; reaching, bending, carrying, and stooping within the daily routine of tasks and activities

ADDITIONAL REQUIREMENTS

This position is contingent upon:

  • Satisfactory results of a Live Scan (criminal background investigation)
  • Successfully obtaining jail clearance for positions working in county jails or probation locations
  • Submitting proof of clear TB results. Results dated within 60 days prior to hire date are acceptable

WORK ENVIRONMENT

  • Must be willing and able to work in a correctional facility and travel throughout the area to community sites
  • Work environment varies from site to site
  • There is some personal safety risk working with the at-risk population
  • Moderate noise level
  • Travel to multiple sites required
