Skye Canyon Post Acute is looking for a full time Social Worker to join our team!

Purpose: To enhance the quality of life to our residents by providing medically-related social services which assists residents in maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs.

Duties and Responsibilities:

• Ensure the needs of our long-term care residents are met
• Provide or arrange provision of needed counseling services
• Complete initial assessments
• Implement care plans
• Assist with discharge planning, i.e., appropriate placement and arrangements, home health services, transportation
• Maintain a community resource list
• Document patient progress
• Coordinate family meetings
• Ensure regulatory compliance
• Other duties as assigned to meet the purpose of this role

Requirements and Qualifications:

• Current SSD Certificate and LCSW Preferred
• Proven experience in Skilled Nursing Facility

Physical Capacities: (With or Without the Aid of Mechanical Devices)

• Must be able to move intermittently throughout the workday.
• Possible bending, squatting, twisting and reaching
• Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met
• Must meet the general health requirements set forth by the policies of this facility
• Must be able to stand for and walk for extended periods of time
• Must be able to cope with the mental and emotional stress of the position

Environmental Conditions: Inside work, normal temperatures, some noise, occasional fumes/odors, possible mild chemical exposure and minimal potential exposure to blood-born pathogens.
