Job description

Heavens Angel Homecare is seeking a professional, compassionate, and detail-oriented Care Coordinator to join our growing team. This position plays a vital role in ensuring our clients receive consistent, high-quality care and support, while also providing guidance and assistance to our caregiving staff.

Key Responsibilities:

  • Serve as the primary liaison between clients, caregivers, and the agency.
  • Conduct initial assessments and follow-up visits to ensure client satisfaction and well-being.
  • Develop and maintain individual care plans in collaboration with clients, families, and care teams.
  • Update client records accurately and promptly in agency systems.
  • Address client or family concerns with professionalism and empathy.
  • Participate in team meetings and contribute to overall agency improvements.
  • Collaborate with healthcare providers, case managers, and other stakeholders to coordinate care.

Qualifications:

  • High school diploma or equivalent (Associate or Bachelor’s degree in healthcare or related field preferred).
  • Experience in home care, healthcare, or case management (1-2 years preferred).
  • Strong organizational and time-management skills.
  • Excellent interpersonal and communication skills
  • Proficient in Microsoft Office and General Computer Use
  • Ability to work independently in a fast-paced environment
  • Compassionate, patient, and client-centered mindset.
  • Knowledge of state home care regulations is a plus.

Job Type: Full-time

Pay: $16.00 per hour

Expected hours: 35 per week

Schedule:

  • Monday to Friday
  • No weekends

License/Certification:

  • Driver's License (Required)
