Perinatal Family Support Specialist MSW – Perinatal Family Support Center - Days
Position Highlights:
Position: Perinatal Family Support Specialist
Location: Evanston Hospital
Full Time/Part Time: Part-Time; 24 hours
Hours: Monday-Friday, 8:30am-5:00pm
Required Travel: yes, Highland Park Hospital
Provides clinical social assessment and intervention including post-discharge plan, as indicated. This position supports at-risk and high-risk pregnant women, pediatric patients, neonates, perinatal and pediatric loss patients, adoption and surrogacy families and obstetrical and gynecological surgery step-down discharges. The objectives of this position support improved patient outcomes, safety and appropriate and timely treatment and discharge.
What you will do:
Perform initial evaluation, assessment, develop interventions and treatment plan
Perform electronic medical record chart review and, when indicated, obtain relevant information from medical team related to patient care
Allocate time, resources and patient communication appropriately based on assessed need
Perform thorough assessment to gather information related to: patient’s emotional and cognitive status, current and/or historic mental health concerns, perception of diagnosis of self and/or fetus/neonate, coping skills, support system and financial status
When indicated, assess substance abuse, intimate partner violence, child abuse/risk of harm/neglect, per NorthShore and Department policies.
Assist with adoption, gestational carrier or surrogacy planning, when requested
Provide socio-emotional and environmental support services, referring to community resources, as needed
Modify treatment plan to reflect patient/family needs, as appropriate
Work collaboratively with state and local authorities, related to patient/family safety, as needed
Use interpretive services, when indicated
What you will need:
Education: Two years clinical experience required, 3 - 5 years preferred.
Certification: LSW, LCSW, LPC, LCPC, PsyD, PhD-Clinical or equivalent.
Experience: Two or more years experience with high-risk, at-risk pregnant women and families, preferred.
Unique or Preferred Skills: 1) Knowledge of developmental needs of the neonate to one month; 2) knowledge of the needs and strengths of the adult maternity patient: cognitive and emotional during the perinatal period, related to L&D and postpartum; 3) knowledge of the developmental needs and strengths of the teen maternity patient: cognitive, emotional during the perinatal period, related to L&D and postpartum; 4) perinatal loss; and 5) in-patient pediatric patients
