Description

SUD Care Coordinator will provide care coordination to individuals, age 18-25, with a Substance Use Disorder Diagnosis. Care Coordination Services involves deliberately organizing a person's care activities and sharing information among all the participants involved with a person's care to ensure safer and more effective care that meets the goals of the person.

Position Requirements

High school diploma or equivalent required.Bachelors' Degree and/or Associate's Degree preferred. CSAC preferred. Good computer skills are also essential due to data entry in our electronic health record. Candidate must also possess a valid driver's license and good driving record. Good organizational and time management skills are crucial. Candidates must pass all CMCSB background screens.

About the Organization
CMCSB was formed in 1972 as a Chapter 10 Community Services Board under Virginia law. CMCSB provides mental health, intellectual disabilities and substance abuse treatment services to the residents of Buchanan, Russell, and Tazewell Counties. At the present time, CMCSB operates over twenty locations, to include outpatient offices, mental health recovery programs, group homes and day support programs for intellectually disabled individuals, and a residential alcohol and drug treatment program. In addition, CMCSB has several services that are provided directly in an individual's home or in a supported environment. CMCSB also provides a 24-hour crisis team to assist community agencies and individuals in crisis situations. CMCSB employs approximately 500 employees, which include full-time and part-time employees, as well as individuals receiving services who are employed to work in various day programs.
