The Foster Care, Care Coordinator is responsible for using trauma-informed evidence-based approaches with members and caregivers for children and youth who are in out of home placement through the foster care system with complex/high or moderate needs who are not enrolled in a home and community-based waiver, on a wait list or receiving services through a Certified Community Behavioral Health Clinic (CCBHC).


This is a field-based role located in Sedgewick, Harvey, Butler or Sumner County, KS and surrounding areas.

Primary Responsibilities:


  • Assess, plan and implement care strategies that are individualized for children and youth in a family-oriented context (e.g., considering reunification and/or other permanency planning needs, needs for adoptive children, and needs for transition-age youth with alternative permanency plans) and directed toward the most appropriate, lease restrictive level of care




  • Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral and medical services




  • Manage the care plan/plan of service throughout the continuum of care as a single point of contact




  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members




  • Advocate for children, youth, and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team




  • Provide trauma-informed and disease/disorder specific education to members and caregivers




  • Coordinate with the child welfare and juvenile justice system on behalf of members and caregivers




  • Coordinate with additional services, including but not limited to, educational/educational advocacy, complex behavioral health (PRTF, etc.), services in line with state Family First Prevention and Services Act for children and families, high fidelity wraparound services, early childhood intervention




  • Complex cases will have additional oversight by master's level case manager, including review of assessments and care plans
